Please complete the form in its entirety. DO not leave any fields blank, because it will only delay your application. Please fax a copy of ALL relevant documents (company registration, ID, passport, bank statements, etc.). Business applications must include a contact person (director/member) detail and ID. All 24 month contracts require 3 months bank statements and month-to-month 1 month statements.

Although there are online forms, due to legislation we can not accept online applications until further notice. Please select the type of application below, download the PDF, complete it and fax it back to 086 686 9842 together with the relevant documentation as set out below:

For 24 Month Contracts

INDIVIDUAL APPLICATIONS:

1. A copy of the application form, signed by the subscriber.
2. A copy of their ID
3. Proof of income

The above three items are sufficient ONLY if:

- the individual earns more than R4000
- the individual's banking details are on their salary slip/proof of income and
- the banking details on the proof of income match those on the application form

 If these criteria are not fulfilled, credit vetting will ask for 3 months bank statements.

COMPANY APPLICATIONS:

1. Copy of Directors/members ID
2. CK Documentation
3. Cancelled cheque or 3 month's bank statements
4. Official Order on a Company Letterhead
